Spay/Neuter Clinic Registration

Kitten Division can transport your cat(s) to the clinic on the dates listed below.
NOTE: Kitten Division is NOT a veterinary clinic, we are a shelter that offers transportation for your cat(s) to the low-cost veterinary clinic.
You may also sign-up in person at Kitten Division, 996 Missouri Avenue, St. Robert during our regular business hours of 12 to 6 pm.

If you are signing up more than one cat, a separate form must be completed for each.

Drop off for clinic is from 8:15 to 8:30 am on the clinic date chosen below.
Pick up is on clinic date between 5 and 6 pm.

We only have a limited number of space available on our transport to the clinic and since we have had problems in the past with no-shows, we now require pre-payment to guarantee your appointment. When you click submit, you will be taken to the payment page or you can pay in person at Kitten Division.
By submitting this form, I hereby agree that the procedures noted above have been approved by me. I understand that with surgery and anesthesia there is a risk of illness, injury and/or fatality. I accept this risk and consent to the surgical procedure for the cat noted above. I agree to hold harmless and release from liability TNR A Better Chance dba Kitten Division Adoption Center, Murphy’s Spay and Neuter Clinic, and Korn Surgical Services from any illness, injury or fatality that results from the cat noted above receiving requested treatments. To my knowledge this cat is healthy and has not bitten anyone in the past ten days.
By submitting this form, I agree to indemnify and hold TNR A Better Chance dba Kitten Division Adoption Center, including their volunteers and transporters, harmless for any damaged caused during the transportation of the cat, or for any damages caused by any unforeseeable events including, but not limited to fire, vandalism, burglary, extreme weather, natural disasters, traffic accidents, crate/carrier malfunction, animal escape, or acts of God. I am aware that my cat may be exposed to communicable diseases (including, but not limited to, upper respiratory infection and fleas) during transport.
